Consent for Sharing Health Data with Clinic

This Consent Form outlines how iollo will collect, use, and share your health data with your clinic.1. Data Collection and Use: We collect various types of health data, including but not limited to, your medical history, diagnoses, medications, treatment plans, and lab results. This data will be used solely for the purpose of sharing with your designated clinic.

2. Data Sharing: By agreeing to this form, you consent to the sharing of your health data with your clinic in accordance with California's Health and Human Services Data Exchange Framework. Your data will only be shared for the purposes outlined above and will not be used for any other purposes without your explicit consent.

2a. Clinical Relationships: Data sharing through iollo's platform does not establish, imply, or constitute a doctor-patient relationship with any clinic or healthcare provider. If you are not currently a patient of the clinic receiving your data, sharing your iollo test results and health information with them does not create a medical care relationship. Only direct engagement with a clinic through their standard patient onboarding process can establish a doctor-patient relationship.

3. Data Protection:
We comply with all applicable data protection laws and regulations, including those specific to California. We have implemented robust security measures to protect your data from unauthorized access, alteration, disclosure, or destruction.

4. Your Rights:
You have the right to access your data, correct any inaccuracies, and request deletion of your data. You also have the right to withdraw your consent at any time. However, this will not affect the lawfulness of any processing carried out before you withdraw your consent.

5. Liability:
iollo will not be held liable for any data breaches or unauthorized access to your data that occur at your clinic or any third-party entities that we do not control.
